Community Reintegration Support Services

Community Reintegration Support Services will provide best practice and evidence-based intervention & programming for Phase 2 youth that have been sentenced to Custody & Supervision and need to fulfill the Community Supervision portion of their sentence under the Youth Criminal Justice Act.
The purpose of Community Reintegration Support Services is to provide a community-based intervention program of intensive supervision, support, and rehabilitation that will assist the young person in the transition from custody to community. This service demonstrates that young persons in conflict with the law can be held accountable for their criminal actions and benefit from increased support and supervision while returning to the community.
Community Reintegration Support Services will achieve its purpose by:
- Increasing the supervision & accountability of the young person in the community;
- Improving: school attendance, behaviour, & performance; vocational preparation/job-readiness; and community participation;
- Helping the young person to meet basic need requirements including shelter, housing, clothing and food;
- Improving family relationships through conflict mediation;
- Improving the young person’s cognitive & social coping skills, with a focus on personal responsibility and prosocial accountability;
- Linking the young person with supportive cultural, social services, and recreational resources.
Services Available For Young Persons:
1. Reintegration Plan (at Custody Facility) [Cost: $50.00]
Community Reintegration Support Services will provide support and assistance in the development of the reintegration plan while the youth is at the custody facility. The Community Reintegration Counsellor (CRC) will attend a facility meeting and prepare a summary of recommendations, which will be submitted to Probation & Parole Services.
2. Individual Support Services [Cost: $25:00 per hour]

3. Group Support Services [Cost: $15:00 per hour]
- Anger Management
- Choices
- Victim Awareness
- “Brake” – Anti-Car Theft Group
4. Section-20 School Services [Cost: $60:00 per day]
Young persons will have access to a Section 20 Classroom with a Child & Youth Counsellor and a Teacher (in partnership program with the Hamilton District School Board) - based on space availability.
Individual Community Reintegration Support Services
Activities available to youth as determined through the Reintegration Plan include:
- Educational/Vocational Support (including school mediation & advocacy);
- Job Preparation, Training & Support;
- Family Conflict Mediation;
- Living Skills Training;
- Assistance with Shelter, Housing, Food, Clothing needs, etc.;
- Anger Management counselling;
- Victim Awareness counselling;
- Various Cognitive Behavioural programs focused on specific problems (e.g., car theft, shoplifting, substance abuse, communication skills, stress management, etc.);
- Parenting Support Counselling (for young persons with children) ;
- Peer Mediation (school-based and/or community-based);
- Linkage/Referral to Community Resources (e,g., alcohol & drug counselling, etc.);
- Linkage with Social Supports (e.g., recreation, clubs, youth organizations, sports);
- Cultural Support (e.g., linkage with cultural organizations, mentors, volunteers, spiritual guidance);
- On-Call & Weekend Support (phone consultation to both clients and families around specific problems or crises).

Early Years Resources for Faith Communities is a project funded by the Ministry of Community, Family and Children’s Services under the Early Years Challenge Fund Initiative. Challenge Fund Community Partners who make the delivery of this service possible include:
- Bayridge Family Center,
- The Bridge Church,
- Midwest Center for Stress and Anxiety,
- Gymboree Play and Music,
- The Shepherds Guide ,
- The Crossroads Center,
- Home with a Heart,
- Starbucks Coffee,
- Connect Family Ministries,
- Christian World Community Expos, and
- Affiliated Services for Children and Youth.
EYRFC supports and promotes cross-denominational faith communities in their development and delivery of quality “early years” programs for parents and their young children (birth to 6 years of age). This is accomplished via the:
- development and coordination of a faith community collaboration network (The Community-Serving Ministries Network of Hamilton and District),
- development and delivery of “demonstration site” - Mom’s Time Out … Tots Time In,
- provision of training for faith community leaders and volunteers,
- development and dissemination of parent-child service regulations and guidelines,
- facilitation and mentoring of program set-up for parents and children,
- facilitation of faith community networking and joint service provision with Early Years Centers, Affiliated Services for Children and Youth, the Community and Public Health Services Departments of Hamilton and Halton, and other related services.
The areas of focus for faith community consultation, training, coordination and mentoring are:
- healthy child development,
- developmentally appropriate programming,
- effective parenting,
- assurance of child health, hygiene, nutrition, safety, and welfare,
- support around special child development and parenting needs,
- choosing quality childcare services and
- awareness and utilization of community services.
